Asuhan Kebidanan Berkesinambungan Pada Ny.J Dengan Penerapan Teknik Oketani Breast Massage Untuk Meningkatkan Produksi Asi Pada Ibu Nifas Di PMB Islah Wahyuni Pekanbaru

Abstract
Produksi ASI yang tidak optimal merupakan salah satu permasalahan pada masa nifas yang dapat memengaruhi keberhasilan pemberian ASI eksklusif. Kondisi ini dapat dipengaruhi oleh faktor fisiologis, psikologis, teknik menyusui, dan kondisi payudara. Salah satu intervensi nonfarmakologis yang dapat membantu meningkatkan kelancaran pengeluaran ASI adalah teknik Oketani Breast Massage. Tujuan karya ilmiah ini adalah melaksanakan asuhan kebidanan berkesinambungan (Continuity of Care) pada Ny. J dengan penerapan teknik Oketani Breast Massage untuk meningkatkan produksi ASI pada ibu nifas di PMB Islah Wahyuni Pekanbaru. Metode yang digunakan adalah studi kasus deskriptif dengan pendekatan asuhan kebidanan berkesinambungan mulai dari kehamilan, persalinan, bayi baru lahir, nifas, hingga keluarga berencana. Penerapan teknik Oketani Breast Massage dilakukan pada hari ke-3 sampai hari ke-10 postpartum melalui wawancara, observasi, pemeriksaan fisik payudara, serta lembar observasi dan checklist. Hasil menunjukkan bahwa sebelum intervensi ASI keluar sedikit dan terdapat sedikit bendungan ASI. Setelah intervensi, ASI lebih mudah keluar, payudara terasa lebih lunak, dan rasa penuh berkurang. Pada hari ke-10 postpartum, pengeluaran ASI lancar, tidak terdapat bendungan maupun benjolan, puting tidak lecet, bayi menyusu lebih efektif, serta tampak tenang dan puas setelah menyusu. Penerapan Oketani Breast Massage membantu meningkatkan kelancaran pengeluaran ASI, kenyamanan, dan kepercayaan diri ibu dalam menyusui.
Suboptimal breast milk production is one of the problems that may occur during the postpartum period and can affect the success of exclusive breastfeeding. This condition may be influenced by physiological and psychological factors, breastfeeding techniques, and breast conditions. One non-pharmacological intervention that may help improve breast milk flow is Oketani Breast Massage. The purpose of this scientific paper was to provide continuous midwifery care (Continuity of Care) for Mrs. J through the application of Oketani Breast Massage to improve breast milk production in a postpartum mother at PMB Islah Wahyuni Pekanbaru. The method used was a descriptive case study with a continuous midwifery care approach covering pregnancy, childbirth, newborn care, postpartum care, and family planning. Oketani Breast Massage was applied from the third to the tenth postpartum day through interviews, observations, breast physical examinations, and standardized observation sheets and checklists. The results showed that before the intervention, breast milk production was limited and mild breast engorgement was observed. Following the intervention, breast milk was expressed more easily, the breasts became softer, and the feeling of fullness decreased. On the tenth postpartum day, breast milk flow was smooth, with no breast engorgement or lumps, and the nipples were not sore or cracked. The baby breastfed more effectively and appeared calm and satisfied after feeding. The application of Oketani Breast Massage helped improve breast milk flow, maternal comfort, and maternal confidence in breastfeeding.
