Abstract

Ineffective airway clearance is the inability to clear secretions or airway obstructions to maintain airway patency (Tim Pokja SDKI DPP PPNI, 2016). This study aimed to determine the application of the Active Cycle of Breathing Technique (ACBT) in addressing ineffective airway clearance in a patient in the Dahlia Ward of Arifin Achmad Regional General Hospital, Riau Province. This study employed a case study approach involving the application of the Active Cycle of Breathing Technique (ACBT) to address the nursing problem of ineffective airway clearance in one patient who met the predetermined criteria. The ACBT intervention was performed once daily for 12 minutes over four days of hospitalization. The instrument used was an observation sheet developed based on the Indonesian Nursing Outcome Standards. The results showed improvements in the patient's airway clearance and respiratory pattern. Effective coughing ability increased from a score of 1 to 4, sputum production from 2 to 3, and rhonchi from 2 to 3. Improvements were also observed in dyspnea and orthopnea, which increased from a score of 2 to 4, while respiratory rate and respiratory pattern improved from scores of 3 and 2, respectively, to a score of 4. These findings indicate that the application of ACBT had a positive effect on the patient's ability to clear the airway and improve respiratory function. The application of ACBT may be considered as an evidence-based nursing intervention for patients with ineffective airway clearance, particularly those experiencing sputum retention or difficulty in expectorating sputum.
