Penerapan Berkumur Larutan Madu Untuk Menurunkan Nyeri Mukositis Oral Pasien Ca Mammae Post Kemoterapi Di Ruangan Dahlia RSUD Arifin Achmad Provinsi Riau

Abstract
Salah satu tatalaksana pasien ca mammae adalah kemoterapi. Efek samping yang ditimbulkan adalah nyeri mukositis. Nyeri yang terasa adalah nyeri sedang hingga berat. Penggunaan terapi non farmakologis juga merupakan salah satu tindakan mandiri seorang perawat dalam mengontrol rasa nyaman yang ditimbulkan oleh nyeri mukositis pada pasien post kemoterapi. Hasil observasi di ruangan Dahlia RSUD Arifin Achmad Pekanbaru masih kurangny penerapan terapi non farmakologis untuk menurunkan derajat nyeri mukositis oral. Minimnya penanganan nyeri dapat mempengaruhi Quality Of Life dan rasa ketidaknyaman pasien. Salah satu terapi komplementer untuk menurunkan nyeri mukositis oral adalah berkumur larutan madu. Madu mempunyai kandungan enzim invertase, diastase, katalase, oksidase dan peroksidase. Enzim oksidase mengubah glukosa menjadi glukonolakton yang menghasilkan asam glukonat dan hidrogen peroksida. Hidrogen peroksida bertindak sebagai agen antibakteri. Jumlah pasien 1 orang, intervensi dilakukan selama 3 hari dengan durasi berkumur 1 menit dilakukan 2 kali sehari pagi dan malam sebelum tidur menggunakan madu 15 ml + air matang sebanyak 50 ml dan skala nyeri diukur menggunakan NRS (Numeric Rating Scale) 0-10. Evaluasi dari intervensi ini didapatkan penurunan derajat nyeri mukositis oral pada pasien ca mammae post kemoterapi dari skala nyeri 5 ke skala nyeri 3. Diharapkan hasil inovasi dijadikan sebagai salah satu tindakan mandiri perawat untuk mengatasi masalah nyeri mukositis oral pada pasien post kemoterapi.
One of the treatments for ca mammae patients is chemotherapy. The side effect that occurs is mucositis pain. The pain that is felt is moderate to severe pain. The use of non- pharmacological therapy is also one of the independent actions of a nurse in controlling the feeling of comfort caused by mucositis pain in post-chemotherapy patients. Observation results in the Dahlia room at Anfim Achmad Hospital Pekanbaru There is still a lack of application of non-pharmacological therapy to reduce the degree of oral mucosal pain. Minimum pain management can affect the patient's quality of life and discomfort. One of the complementary therapies to reduce the pain of oral mucositis is a honey solution. Honey contains the enzymes invertase, diastase, catalase, oxidase and peroxidase. The oxidase enzyme converts glucose into gluconolactone which produces gluconic acid and hydrogen peroxide. Hydrogen peroxide acts as an antibacterial agent. The number of patients was 1 person. The intervention was carried out for 3 days with 1 minute gargling duration, 2 times a day, morning and evening before bed, using 15 ml of honey, 50 ml of boiled water and the pain scale was measured using NRS (Numeric Rating Scale). 0-10. Evaluation of this intervention found a reduction in the degree of oral mucositis pain in post-chemotherapy ca mammae patients from a pain scale of 5 to a pain scale of 3. It is hoped that the results of the innovation will be used as one of the nurses' independent actions to overcome the problem of oral mucositis pain in post- chemotherapy patients.
