Abstract
Penderita Diabetes Melitus (DM) sebagian besar mengalami komplikasi dan salah satu yang terburuk adalah infeksi pada kaki . Ulkus Diabetikum/Luka Diabetik adalah kematian jaringan yang disebabkan oleh penyumbatan pembuluh darah (ischemic necrosis) karena adanya mikroemboli aterotrombosis akibat penyakit vaskular perifer oklusi yang menyertai penderita diabetes sebagai komplikasi menahun dari diabetes. Dengan adanya ulkus yang terinfeksi, maka resiko amputasi menjadi lebih besar besar. Tingkat amputasi tungkai bawah 15 kali lebih tinggi pada penderita diabetes dibandingkan dengan non-penderita diabetes. Penyakit ini dapat ditangani dengan pengobatan farmakologi ataupun nonfarmakologi. Salah satu pengobatan nonfarmakologi yang dapat menggunakan yaitu Aloe vera gel. Tumbuhan ini mampu merangsang proliferasi sel, meningkatkan produksi kolagen dan koneksi transversal sehingga mempercepat penyembuhan luka, serta dapat digunakan untuk menjaga keseimbangan kelembaban. Tujuan dari asuhan keperawatan ini adalah untuk menerapkan Penerapan Exstrak Gel Lidah Buaya Pada Pasien Ulkus Diabetikum Dengan Resiko Infeksi Di Ruang Krisan RSUD Arifin Achmad. Metode yang digunakan adalah observasi langsung dengan menggunakan lembar observasi Bates-Jensen Assesment Tool Skala Bjwat. Asuhan keperawatan dilakukan mulai tanggal 29 hingga 31 mei 2024. Hasil evaluasi menunjukkan bahwa terdapat perubahan atau penurunan skor lembar observasi dari wound status continuum 33 menjadi wound status continuum 25 dimana luas luka berkurang, pus tampak berukurang, luka tepi menjadi merah jambu.
Diabetes mellitus (DM) sufferers mostly experience complications and one of the worst is infection of the feet. Diabetic Ulcers/Diabetic Wounds are tissue death caused by blockage of blood vessels (ischemic necrosis) due to atherothrombotic microemboli due to occlusive peripheral vascular disease that accompanies diabetics as a chronic complication of diabetes. With an infected ulcer, the risk of amputation becomes greater. The rate of lower limb amputation is 15 times higher in diabetics compared to non-diabetics. This disease can be treated with pharmacological or non-pharmacological treatment. One non-pharmacological treatment that can be used is Aloe vera gel. This plant is able to stimulate cell proliferation, increase collagen production and transverse connections thereby accelerating wound healing, and can be used to maintain moisture balance. The aim of this nursing care is to apply the application of Aloe Vera Gel Extract to Diabetic Ulcer Patients with a Risk of Infection in the Chrysanthemum Room at Arifin Achmad Hospital. The method used is direct observation using the Bates-Jensen Assessment Tool Bjwat Scale observation sheet. Nursing care was carried out from 29 to 31 May 2024. The evaluation results showed that there was a change or decrease in the score on the observation sheet from wound status continuum 33 to wound status continuum 25 where the area of the wound decreased, the pus appeared sized, the wound edges became pink.
