Abstract

Pemberian ASI eksklusif pada bayi baru lahir merupakan upaya penting dalam mencegah kematian dan masalah kekurangan gizi. World Health Organization (WHO) melaporkan cakupan pemberian ASI eksklusif global baru mencapai 44% pada tahun 2023. Salah satu kendala pemberian ASI adalah kondisi puting susu tenggelam (inverted nipple) yang sering terjadi pada masa nifas. Penelitian ini bertujuan memberikan asuhan kebidanan kepada Ny. Y, ibu nifas usia 41 tahun dengan riwayat obstetri dua kali melahirkan, nol abortus, dan dua anak hidup (P2A0H2) pada hari pertama masa nifas, untuk mengatasi puting susu tenggelam melalui teknik pijat hoffman dan penggunaan spuit terbalik di PMB Bd. Hj. Winalti, SST, Pekanbaru. Studi kasus dilakukan selama 4 hari (24–27 Mei 2025) dengan pendekatan SOAP. Intervensi berupa edukasi, demonstrasi pijat hoffman, dan penggunaan spuit terbalik diberikan setiap hari. Hasil menunjukkan perbaikan puting mulai tampak menonjol pada hari ke-3, bayi dapat menghisap dengan baik, dan pengeluaran ASI mulai lancar.

Exclusive breastfeeding for newborns is an important effort to prevent death and malnutrition. The World Health Organization (WHO) reports that global exclusive breastfeeding has only reached 44% in 2023. One of the obstacles to breastfeeding is the condition of inverted nipples which often occurs during the postpartum period. This study aims to provide midwifery care to Mrs. Y, a 41-year-old postpartum mother with an obstetric history of two deliveries, zero abortions, and two living children (P2A0H2) on the first day of the postpartum period, to overcome inverted nipples through the Hoffman massage technique and the use of an inverted syringe at PMB Bd. Hj. Winalti, SST, Pekanbaru. The case study was conducted for 4 days (24–27 May 2025) with the SOAP approach. Interventions in the form of education, forced Hoffman massage, and the use of an inverted syringe were given every day. The results showed that the nipples began to appear prominent on the 3rd day, the baby could suckle well, and the milk flow began to flow smoothly. Mothers' knowledge increased from the category of "less" (31%) to "good" (87%). In conclusion, Hoffman massage and inverted syringes are effective in overcoming inverted nipples and increasing maternal independence in breast care.
